PLEASE READ TO YOUR STUDENTS AT THE END OF FIRST PERIOD
*Student and parent information on sports and other school physicals:
Free sports physicals will again be offered at the Fremont Area Medical Center Health Park Plaza. The time for Dodge 9-12 students has been set for June 7th at 1:00 PM.
Dinklage Medical Clinics will no longer offer “mass physical days”. Beginning May 19, they will offer school physicals for $35.00 with a scheduled appointment. (must schedule as a school physical in order to get this price.) Further information can be obtained by calling one of the clinics.
Both clinics will use the standard NSAA forms – you can obtain a copy of these forms in the school office. There is a pre-participation physical evaluation history form that should be completed and signed by student and parent BEFORE you go to the exam and it should be taken with you. The physical evaluation and clearance form should be taken and filled out by the doctor. The final form is the student and parent consent form. This form does NOT go to the clinic – it is filled out and signed by parent and student and should be returned to the school office before the first practice of the season.
Please remember that physicals for 7th graders is a different form. Those were mailed previously to all 6th grade students.
