Physicals Need To Be Completed Before First Day of Practice and School

High School and junior high students planning to participate in athletics are required to have a physical examination completed before they can practice.  Physical forms may be picked up in the office.  

Seventh grade students must have a completed physical as well as proper immunizations.  Kindergarten students must have a physical, proper immunizations, and an eye examination.  If you need a physical form please stop by the principal’s office for a copy to take to your doctor appointment.  Completed physical forms can be returned to the office.
